The Myth of Strength and Pain: Why Stronger Isn’t Always Better

“If I can just make my core stronger, my back pain will go away.”

It’s one of the most common beliefs I hear in my office.

For years we’ve been taught that pain comes from weakness. If your back hurts, strengthen your core. If your knee hurts, strengthen your quadriceps. If your shoulder hurts, strengthen your rotator cuff.

Strength certainly has value.

But strength alone is not the answer.

In fact, one of the biggest misconceptions in musculoskeletal healthcare is believing there is a direct relationship between how strong you are and how much pain you experience.

There isn’t.

Is a weak person always in pain?

Does a strong person not have pain?

How about a body builder, no pain??

Of course, the concept drawn out a bit begins to show its unraveling.

Pain Does Not Measure Damage

One of the most surprising discoveries in modern pain science is that pain and tissue damage do not always match.

People can have severe arthritis with very little pain.

Others can have debilitating pain with relatively minor findings on an MRI.

The brain is constantly evaluating far more than the condition of the tissues themselves. It considers previous injuries, movement confidence, inflammation, sleep quality, stress, fear, balance, coordination, sensory information, and thousands of incoming signals every second.

Pain is not simply a measure of injury.

It is the brain’s perception of how much protection your body needs at that moment.

Strength Is Only One Piece of the Puzzle

Imagine shopping for a sports car.

Would you buy one based solely on horsepower?

Of course not.

A great sports car doesn’t simply go fast.

It has steering, suspension, traction, braking, balance, timing, responsiveness, and control.

Without those systems working together, more horsepower simply makes the car harder to drive.

Your body works the same way.

Muscles generate force.

But your nervous system determines whenhow muchhow quickly, and in what sequence those muscles should contract.

Without good control, more strength does not necessarily create better movement.

Sometimes it simply creates stronger compensation.

Your Brain Is the Real Control Center

Every movement begins in the brain.

Before you bend over, climb stairs, throw a ball, or reach overhead, your brain predicts what should happen.

It uses information from your eyes, inner ear, muscles, joints, skin, and previous experiences to create a movement strategy.

If the brain no longer has confidence in a joint because of injury, pain, surgery, or prolonged inactivity, it often changes the way that joint moves.

Sometimes muscles tighten.

Sometimes they weaken.

Sometimes they activate too early.

Sometimes they activate too late.

The result isn’t necessarily a lack of strength.

It’s a loss of efficient communication.

More Strength Doesn’t Always Mean Better Function

This is why someone can have:

  • Strong abdominal muscles but chronic low back pain.
  • Powerful quadriceps yet persistent knee pain.
  • Excellent shoulder strength while continuing to struggle with impingement.
  • Elite athletic performance and still develop repetitive injuries.

The issue isn’t always how much force the muscles can produce.

The issue is whether the entire movement system is functioning together.

The Goal Is Better Movement

Movement is medicine.

But not because movement simply builds muscle.

Movement teaches the brain.

Every comfortable, coordinated, confident movement provides valuable information to the nervous system.

The brain learns:

“This movement is safe.”

“This joint is reliable.”

“I don’t need to protect this area so much.”

Over time, the nervous system often becomes more efficient, movement becomes smoother, and many people naturally regain strength as a result.

Rehabilitation Should Restore Function, Not Just Build Muscle

One of the primary goals of rehabilitation should be restoring the quality of movement.

That includes improving:

  • Joint mobility
  • Coordination
  • Balance
  • Timing
  • Body awareness (proprioception)
  • Confidence in movement
  • Endurance
  • Appropriate strength

Strength is still important.

It simply isn’t the whole story.

Think Function Before Force

The healthiest joints aren’t always the strongest joints.

They’re the joints that communicate effectively with the brain.

They move efficiently.

They adapt to changing environments.

They distribute forces appropriately.

They recover after stress.

Strength supports this system—but it doesn’t replace it.

The Bigger Picture
